FIQ-R questionnaire — fibromyalgia's impact on functioning
An interactive version of the FIQ-R (Revised Fibromyalgia Impact Questionnaire), which measures how much fibromyalgia affects a patient's daily functioning. It has three parts: function, overall impact and symptoms. The result (0–100) is calculated automatically at the end.
The tool is for monitoring symptom severity in people already diagnosed with fibromyalgia — it is not a diagnostic test. It takes about 30 minutes on average, but you can pause and return at any time.
Before you start
- Answer based on how you have felt over the last 7 days.
- For each question pick a value from 0 to 10 that best describes your situation.
- If you cannot perform a given activity, select 10.
- If you did not perform an activity in this period, rate the difficulty the last time you did.

How to interpret the score
0–30 · Low impact
A mild impact of fibromyalgia on daily life. Fewer or milder symptoms allow relatively good functioning.
31–59 · Moderate impact
A moderate impact of fibromyalgia. A noticeable level of pain and other symptoms that moderately affect daily activities.
60–100 · High impact
A significant impact of fibromyalgia. Often considerable pain, fatigue and other symptoms that seriously disrupt daily tasks and quality of life.
The FIQ-R is just one tool for assessing fibromyalgia's impact. The condition is complex and its picture can differ markedly between individuals. A physician decides on disease severity and treatment, taking into account overall health, medical history and individual circumstances.
